Applying for a Declarations of Value
The Consulate of Italy in Detroit can issue Declarations of Value for duly recognized Academic and Professional titles conferred in the following states only:
Michigan, Ohio, Indiana, Kentucky, Tennessee.
For all other states, please contact the appropriate Consulate/Embassy of Italy of jurisdiction.
To request a Declaration of Value, please submit the following documents:
- Declaration of Value Application Form;
- Photocopy of valid passport and drivers license;
- Original Diploma and official final Transcripts, authenticated by a school official in the presence of a Notary Public, and Apostilled by the Secretary of State;
- Separate Italian translation of the Diploma and Transcripts;
- Prepaid and Self Addressed return envelope with tracking.
Declarations of Value requested for the purposes of continued education/university admittance in Italy will not incur a Consular Fee.
