Visit of a Consular Representative in Cleveland, Ohio

Please be informed that a consular representative will be in Cleveland, OH for the acquisition of fingerprints and collection of Italian passport applications on July 7, 2022. Residents of Ohio registered in this Consulate and registered in the AIRE (Registry of Italians Residing Abroad) will be able to have their fingerprints taken for the issuance of their passports at Esq. Ezio Listati’s office at 1282 West 58th Street, Cleveland, Ohio 44102. between 10:30 am to 3:30 pm.

Nationals will have to make their appointments by email through our passport office. Appointments will be scheduled until the maximum number of available spots will be reached.

Please bring the following documents to your appointment:

· Application Form, duly completed and signed;

· For the issuance of passports to adult applicants with minor children, consent form (atto di assenso) and a copy of a valid ID from the other parent (if the other parent is not an Italian citizen, he/she must sign in front of a public notary);

· Current Italian Passport (if you are applying for your first Italian passport, or if you also have US citizenship, please include a copy of the first two pages of your US passport);

· If you are a resident of the US, but not a citizen, copy of US Green Card or of valid US visa;

· 2 (two) photographs, passport size (full-face, must have a WHITE BACKGROUND);

· A money order made out to the Consulate of Italy in Detroit for $135;

· A self-addressed and prepaid envelope (USPS Express Mail or similar) for the restitution of the new passport.
